Updated guidance published for avoiding unplanned admissions

The update to the guidance is to correct mistakes found on page 7 and page 15 respectively and to add a new footnote to page 7. Initial patients added to the register should have a care plan in place by ‘the end of September 2014’ and not as previously written ‘the end of June 2014’. Initial patients added to the register, who are under 75 years of age, should be informed of their named GP by ‘the end of July 2014’ and not as previously written ‘the end of June 2014’. The new footnote clarifies that any initial patients added to the register who are over 75 years will already have been informed of their named GP, as per the requirements for Named GP for over 75s.
