The information on the certificate should include:
- the consultant’s name and identity details, including GMC registration number and specialty
- contact details of a named person in the employing organisation – usually the medical director or delegated clinical director
- contact details of a named person in the employing organisation's HR department
- confirmation of validity and dates of most recent CRB and occupational health checks which have been carried out in the consultant’s home trust (supporting original documentation will not be necessary).
