Building a positive culture around exception reporting

In this case study we look at how University Hospitals of Derby and Burton NHS Foundation Trust embedded the process of exception reporting, resulting in a safer working environment and a better educational experience for junior doctors.

Since the implementation of the 2016 terms and conditions of service for doctors and dentists in training, the trust has:

• developed trust specific guidance
• held feedback sessions with junior doctors
• sent out monthly newsletters including ‘you said, we did’ updates
• held training sessions for educational supervisors.
