Model declaration forms outline a range of questions that employers may ask of applicants to help them in their assessment of suitability for an NHS position.

This form should only be used when recruiting to positions which fall under the provisions of the Rehabilitation of Offenders Act and therefore are not eligible for a standard or enhanced check (known as non-exempt positions).
