Updated model declaration forms

NHS Employers has updated model declaration forms A and B to provide further clarity, including signposts to support on self-declaration.

NHS Employers has updated model declaration forms A and B to provide further clarity on who might be deemed as an investigatory body, and to better signpost to where applicants can receive support on self-declaration. 

Details of what has changed:

  • We have added an NHS organisation's local counter fraud specialist to the list of example investigatory bodies (question five on both forms).
  • Both forms signpost applicants to guidance on self-disclosure.
  • We have updated the link to Unlock guidance so applicants can click straight through to the relevant part of Unlock's website.
