Updated model declaration forms

NHS Employers has updated model declaration forms A and B to provide further clarity, including signposts to support on self-declaration.

4 February 2021

NHS Employers has updated model declaration forms A and B to provide further clarity on who might be deemed as an investigatory body, and to better signpost to where applicants can receive support on self-declaration. The updated forms can be found on our website and should be used from today.

Details of what has changed:

  • We have added an NHS organisation's local counter fraud specialist to the list of example investigatory bodies (question five on both forms).
  • Both forms signpost applicants to guidance on self-disclosure.
  • We have updated the link to Unlock guidance so applicants can click straight through to the relevant part of Unlock's website.

Further information

Model declaration forms outline a range of questions that employers may ask of applicants to help them in their assessment of suitability for an NHS position. Employers may find it helpful to read our guidance on self-declaration to understand when and how to seek a self-declaration from applicants.
