Model declaration forms outline a range of questions that employers may ask of applicants to help them in their assessment of suitability for an NHS position.
The form should only be used when recruiting to positions that are listed in the Exceptions Order of the Rehabilitation of Offenders Act or the Police Act regulations as being eligible for a standard or enhanced disclosure through the DBS.
