Updated medical and dental pay and conditions circular
Updated pay figures and mirrored nodal points, as a result of the nodal point reform.
As a result of the nodal point reform, this new pay and conditions circular amends the pay figures for doctors and dentists in training and the mirrored nodal points for locally employed doctors, from 1 April 2026, as agreed between the government and the BMA.
The new pay structure and pay values are now set out in Annex A: Section 1 of the pay and conditions circular (MD) 1-2026R2.
Terms and conditions (TCS) will be updated in due course to reflect the following changes:
- 0n call and weekend allowances have been recalculated in line with the new structure and pay values.
- The nodal pay point system was restructured from five points to ten points to align with stages of training. The MT01-05 pay scale has been expanded to add five additional scale points. Employers should review these posts as any relevant changes will need to be applied manually.
- Flexible pay premia table 1 has been amended to reflect the agreement that the GP flexible pay premium will no longer be defined as a hard-to-fill programme supplement and has been renamed the general practice registrar enhancement. The table has also been realigned to make clearer that histopathology is defined for these purposes as a hard-to-fill training programme.
- Whilst version 14 of the 2016 TCS remains in force, all elements of paragraphs schedule 2 paragraphs 21 to 62 which apply to the GP flexible pay premium as a hard-to-fill programme supplement should be read as applying to the general practice registrar enhancement. This will include pay protection arrangements for doctors changing specialties or returning to training.
Please note: pay changes and arrears will be reflected in September salaries.
